evaluation: Have you had an emg/ncv study or ssep study to determine the nerve involved? Could be long thoracic or spinal accessory nerve, I have seen both .See a good interventional pain physician to rule out intercostal neuralgia, myofascial pain (serratus/ rhomboid), thoracic disc herniation, paravertbral neuralgia, or suprascapular neuralgia.

Neck or scapula: A thorough physical exam and history to determine if it is scapular pain or referred pain. A neck disc is commonly referred to scapula, and an MRI is sometimes diagnostic.
