One has to think that it is more likely neoplasm and of those it is the squamous Ca of the scalp that can cause a non nealing rash and spread to regional cervical lymph nodes.  Bx of course is indicated with initiation of proper treatment.

Unrelated: Scalp rash and joint disease are not symptoms of HIV. Both symptoms may or may not be related. Some connective tissue disorders like lupus may be manifested with both.  Obviously,  you need to be examined by a physician who would also order the appropriate tests
