No. : A transient ischemic attack is due to a temporary (usually 1 hour or less) episode of neurological dysfunction of part of the brain( different symptoms such as numbness, weakness, speech or vision problems depending upon the part affected) due to ischemia (decreased blood supply) which can be caused by a clot in the blood vessel or a clot from somewhere else such as the heart or the carotid artery.

Need further Eval: Ocular migraines are a diagnosis of exclusion--by which I mean that there cannot be another explanation for the changes in vision reported.  I am concerned about your subcortical changes on MRI and numbness, which could be the result of another neurologic diagnosis such as MS.  Please strongly consider an evaluation with a neurologist sooner rather than later....  Take care and stay healthy!

Tingling L shoulder: Could be thoracic outlet syndrome  or coming from compressed irritated nerve in the neck. Would get MRI of cervical spine and if normal follow up with  orthopedist regarding  thoracic outlet syndrome or other impingement disorders, rarely could be MS or mass(tumor- check CXR, thyroid, nodes) or TMJ . Would not delay as over time  with tingling can lose  function and strength.  ,

Get medical help now: You should go to an ER or urgent care ASAP. You may have suffered a spinal cord injury or an injury to the autonomic nerves to your pelvic organs. You need to be evaluated and treated before the injury gets worse.

Whether brain lesion: ..affect both sides of the body or just one side depends on its size and location. In the cerebral hemispheres a small lesion may cause sensation changes on the opposite side of the body. In contrast, lesions in the brain stem or spinal cord may be very profound and affect both sides of the body leading to complete sensory loss or paralysis. Tingling is more likely due to peripheral nerve problem.

Exam needed: Vit D deficiency may be associated with a general increase in pain perception, but you described a very isolated pain syndrome which requires investigation. Call your FP, PMR, Neurologist for an exam to determine where the problem stems and get non invasive testing (EMG, NCS, MRI) for an accurate diagnosis. Good luck

Sometimes there is : Numbness present from the nerve being compressed by the disc herniation that can resolve or persist after surgery ; sometimes the surgery leads to inflammation of the nerve which can lead to numbness that can also resolve or persist in the area that nerve supplies but if you have one sided body numbness you should go immediately to the er to make sure there is nothing else happening
