It is treated with nitrates and beta blockers to try and reorder supply and demand. Stress induced peripheral ischemia is treated with peripheral vasodilator medications.

Tables give the predicted max heart rate response, and tell if it was a maximal stress test.

Ischemia: This is usually used to describe results of a stress test which means that  with certain provocation (exercise or chemical) you have ischemia ( lack of blood flow ) to a part of the heart called the anteroseptal wall usually served by the left anterior descending artery.

Technically: Difficult suggests anatomic artifacts or structures obscuring the view, typical for fat tissue. If the doctor cleared it in the context of your symptoms and other tests, that should be reassuring for you. If follow up was recommended with or without tests, that was not for having an abnormal test, but due to the  need for surveillance given your overall medical history.

Not necessarily: You need confirmatory testing for this abnormal finding.  A high grade blockage in a heart artery could cause this even if you have not had a heart attack.

Reversible defect?: If you're describing what we term a "reversible defect", wherein the heart seems OK at rest and not OK when stimulated (by exercise or medication), then this would mean that area of the heart is still vulnerable to damage (heart attack).  This needs to be addressed and treated.  You may need cardiac catheterization to define the anatomy of the vessel supplying that area with blood.
