: Burning, Itching, Rash and Discharge can all be symptoms of a Sexually Transmitted disease. You should see your Dr for a culture and testing so that you can receive prescription medication to treat the infection. If left untreated, STD's can cause long term problems including infertility. Make sure you use a condom every time you have sexual intercourse to prevent contact with bodily fluids.

Urethritis: It surely sounds like you have a prostate or urethral infection. If you have been sexual then the discharge is either from gonorrhea or chlamydia or both. The dots could represent other infections or a reaction to infection. You need testing and treatment.

Pain fiber stimulation can be experienced as burning, sharp, tingly, itchy, piercing, stabbing, etc  the main point here is that this 50 y/o male is experiencing subjectively a sensation in the genitals. The causes could be many! including psychogenic, diabetic neuropathy, other forms of peripheral neuropathy, prostate problem, herpes, etc.
