Q: can a good d-dimer blood test result help exclude an abdominal aortic aneurysm?

No: At this stage, the use of d-dimer levels for AAA screening is experimental and may be used in conjunction with other screening modalities (ultrasound / ct scans). It would not be safe to only use d-dimer levels to screen for aaa's.

Q: d dimer test results is high. ultrasound normal, vq scan normal, thigh is still swollen and painful what would be next step?

Swollen Thigh: Mri or ct scan of the pelvis and thigh.

Why the test?: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, an elevated D-dimer is not diagnostic of DVT, a negative is of more value in excluding DVT. Wish you good health!

: D-dimer is more useful in excluding blood clots. A level of 1.5 may or may not be high depending on the reference range of the laboratory that did the test. There are many causes of elevated d-dimer, eg., inflammation, trauma, pregnancy etc. Talk to the doctor who ordered the test and knows more about your health! Wish you good health!

Positive not useful: A negative d-dimer test is very useful--because it practically tells you no clot. A positive test is not very useful because quite a few conditions cause positive d-dimer test: liver disease, high rheumatoid factor, inflammation, malignancy, trauma, pregnancy, recent surgery as well as advanced age can cause high d-dimer test.. Humans are always producing and eliminating small clots, especially in connection with menstruation, so an elevated d-dimer can have multiple reasons. Need further information on why the test was ordered, etc.

Non specific test: I would put more stock in the negative radiology findings. A d-dimer that is slightly elevated is so non-specific that it is really not at all helpful is making a diagnosis of anything.

Many things.: D dimer is only elevated when there is clot breakdown…..But that could occur at that low level from a variety of non serious things. Really need to know why the test was drawn to be able to interpret it properly.
