Secondary causes of erythrocytosis can include:  Poly cystic liver/kidney disease, EPO secreting tumors, obstructive sleep apnea, heart and lung diseases.  In order to determine the cause of elevated hemoglobin a serum EPO level should be performed.

Not likely: Liver cysts are very common (up to 7% of population have them).  Usually these are discovered "incidentally" while looking for other problems.  Simple liver cysts are almost always asymptomatic and would be very unlikely to cause fatigue.  Continue to work with your doctor to find an explanation and possible solution to your fatigue.
