Speculum exam?: Maybe one of the female physicians can answer this better than I, but I would expect that it would be similar to a speculum exam by a gynecologist.  It is a probe, designed to go into the vagina that allows the physician to see what's going on inside your abdomen.  It is much closer to the abdominal structures so tends to give a better view of certain things when transabdominal U/S fails.

Probably not: Any type of ultrasound at any point in pregnancy has a range of error much less than 5 weeks.  At worst a scan in the late third trimester is the least accurate but still within a range of error of 23 days or less. Trans-vaginal pregnancy dating is usually done before 14 weeks and is within 4 to 14 days depending on how far along the pregnancy is.

Vaginal ultrasound: The probe will be inserted into your vagina. Through the crystal at the top of the probe sound waves will be sent and returned, and processed in the machine. The picture that you may see on the screen will be white and black and various shades of grey. The uterus will be white mostly and the bladder will be black. They will look at your tubes and ovaries as well as the uterus.

Not much: It's very early. Usually at 5 weeks all we see is endometrial thickening and maybe a sac, but not always.

Yes: An ultrasound date of 9weeks and 4 days is 'dating' from the last menstrual period forward and conception is about two weeks later (a little more for longer cycles and less for shorter cycles).  Ultrasound in the first trimester also has a range of error of +/_ 5-7 days. In other words, an ultasound date  of 9 weeks and 4 days on10/10 would make the last menstrual period the first week of august.

Likely and unlikely: They will likely show you an ultrasound view of your baby if it is progressing appropriately. The procedure is not usually uncomfortable.

Ultrasound: You should be able to see the baby.  The ultrasound screen is usually right next to the bed.  However, a vaginal ultrasound is used to look at early pregnancies and if less than 6 weeks you may only see early pregnancy signs.
