Not necessarily: The autonomic dysfunction is a nuisance but the major variable is how your lupus is controlled.

i have postural orthostatic tachycardia syndrome and lupus- does that mean the lupus has already attacked my brain with neurologic issues ?! poor prognosis?

It does not mean ..: No it doesn't mean that at all. Their are many ways to assess disease activity in lupus and/or lupus cerebritis.  Postural hypotension is not a feature of lupus cerebritis.  Talk to a rheumatologist about your concerns!

I think it's fair: to say most doctors who went into a medicine career did so to try to save lives, or at least to improve quality of life.  For a doctor to say that a patient has a poor prognosis is an admission to himself/herself that there is not much more doable to save the person from the inevitable jaws of death.  However, having a poor prognosis does not necessarily mean poor quality of life.  So ask about

Guarded prognosis: This typically means that based on the condition we cannot make a clean cut determination as to which way pone could go. Essentially you have a 50/50 chance of getting better or getting worse.

Not necessarily: The ANA titer will not correlate with disease activity.  Your symptoms or what organ systems that are involved early in the disease course can sometimes be predictive of how the disease will be.  If you have severe organ involvement requiring medications such Cytoxan (cyclophosphamide) that can sometimes be associated with a more worse prognosis. I hope that helps.

does a gleason score of 8 mean prognosis is very bad. so much info on internet is very scary and they all seem to give false inaccurate ?

Needs treatment: The Gleason score (grade) assesses how aggressive the prostate cancer is.  8 is on the aggressive side meaning that it has a higher chance of spread outside of the prostate. You need to talk to your urologist about imaging studies to make sure no spread has occurred.  Also this is a prostate cancer that needs treatment (cannot be observed).  Talk to your doc about options

is it true that if you're diagnosed with myocarditis and there is fibrosis you're prognosis is poor and that person has only a 50% chance of surviving 5 years?

Accepted Answer

: Every case is different; it probably is not accurate to generalize to all myocarditis with or without fibrosis. Your cardiologist is the only appropriate source of advice for this question. Good luck.
