i have had a tingerling burning sensation in my back around my shoulder blade on the left side for 3 years on and of now i am a smoker?

Cervical Referral?: For DX purposes the scapula is a BIG area.  Very common is burning pain at inner edge scapula &amp; in the upper thoracic spine. The basic problem is a neck facet arthritis C4-5? that refers pain to that area and is worse with stress &amp; poor posture. Aleve+ sedation+ exercise and sometimes a chiropractor will help. If not this, please be more specific in location and relation to activity &amp; posture

Have your doctor refer you to a back surgeon.

Neuropathy: Make sure that you don't have any skin lesions, this symptoms may be caused by herpez zoster. Also, visit your pcp, you might have a symptoms of cardiac ischemia.

The textbook says...: This type of pain may need a thorough check with an urgent care physician if sudden onset.
If it is chronic, see a doctor to evaluate your neck/ shoulder for a muscle sprain/ neck problems, etc.

Just a thought: but this pain which sound like "nerve pain" may originate from  a pinched nerve in your LOWER NECK! (C6-C7) level or upper back C 7-T1
Ask your Primary Health Care Provider if this is possible and an in person visit may be helpful!

Pinched nerve: Neck pain that runs down the arm or shoulder with or without numbness and tingling sounds like a pinched nerve in the neck. This is usually caused by a protruding disc or bone spur touching one of your spinal nerves as it leaves your neck.  This can often improve on it's own with rest and anti-inflammatories, but if your pain lasts greater than one week or gets worse see your physician.

Possibilities: You might have notalgia paresthetica, but the full description points toward brachial plexus. If your symptoms increase on elevation of your arm, could be thoracic outlet syndrome.  If better on raising arm, might be nerve pinching in neck.

Need to diagnose: You could possess problems in the cervical spine, although some of this is not typical. Tmj dysfnctn could be the culprit, and maybe you have problems in the brachial plexus such as thoracic outlet syndrome.  Consider possible nerve or joint pain generators, and see your doctor to get a confirmed diagnosis first, and treatment later.
