Impotence: Your family physician should be where you start so that the easy and common testing and evaluation can be done. Then it may be on to a urologist if indicated.

I would just also add a caution to you that one should not pre-judge the causes of erectile problems.  Most are about the brain and the mind and not about the body.  Please have an open mind about this.

: Thinning of hair has many causes . You should see your MD for that. The surgery for castration is not done on demand but for a reason or disease. No urologist will do it just because you want it. Sorry

: It depends on your previous PSA level and if you have history of benign prostatic hypertrophy (BPH) or not. The incremental change of PSA level is more important than the absolute number and people with BPH have relatively high PSA. A rectal exam by your family doctor is indicated if you have not had rectal exam for a long time.

Infertility expert: Usually physician ordering semenanalysis can advise in this matter. However you have result of semen analysis so you can consult an infertility specialist. Simplest treatment for low motile sperm count ( number of live wriggling sperm), assuming there are number live sperm present,  is inseminating wife with your sperm directly into cervical os (opening in cervix)

: Not a urologist, but an STD/HIV expert with 50 years professional experience. This is extremely low risk for HIV. It is very unlikely a woman like your partner has HIV. And even if she does, the risk of transmission from a single episode of unprotected vaginal sex is around one chance in 2,500. HIV testing isn't necessary, but you are free to be tested for reassurance in a few weeks.
