i have fecal anti-gliadin antibodies. through blood tests there was no sign of celiac. my secretory iga was a bit low as well. what does this mean? should i be completely gluten free? or just low doses? and what does secretory iga indicate? thank you?

: IgA is the group of antibodies that deal mostly with gut issues. If your natural IgA levels are low, the typical celiac screening blood tests are not reliable for celiac markers. I would discuss this with your treating doc who has your full history/exam and testing data.

my 20 yr old daughter has low iga of 14 (normal range 50-400) and high ige of 323.0 (normal range 2.0-114.0. she has a normal igg and normal igm. we got this ask tested due to vomiting and nausea. what does low iga and high ige indicate?

: low IgA is often an inherited trait with few associated issues. High Ig?E is found in people with allergies/eczema and similar conditions. Finding a high IgE to a specific item like a food may indicate it could trigger a dangerous reaction. Please discuss this with the doc who ordered it to put it in the context of her history and treatment needs.

Infection: Giardiasis and certain viral diarrheal illnesses. Giardia infection of the small intestine can cause decreased absorption of nutrients and may be mistaken for gluten enteropathy. Surprisingly patients with low IgA do not have a higher frequency of food allergy. That's giardia in the photo.

Accepted Answer

Don't worry: Isolated low IgA occurs in about 1/500 people.     If it is the only immunoglobulin being low, i won't be concerned over it.

Gluten sensitivity: Typical findings for gluten intolerance with reduced iron absorption and thus the low ferritin. Need to avoid gluten and take some iron supplement (or even shots)

: Neither. All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. TTg-IgA: The IgA antibody to Tissue Transglutaminase is currently the best blood test to screen for Celiac disease.  If this level is elevated, further evaluation should be considered.

More than one issue: Low B12 can explain the high mch, mcv and low rdw; these are indications of a macrocytic anemia. You may need B12 injections to address the issue. How high is the IgA? Need to know the actual value as there is considerable variation in normal values.

Low IgA and IgG: It depends how low and if she is past infancy and if she is having recurrant respiratory or urinary symptoms. Then she should see an immunologist.

Probably not: Work the up independently. You're unlikely to find why your IgA is low and this is probably not going to be productive. One person in 500 doesn't have any. Iron deficiency, by contrast, is extremely serious. You have colon or stomach cancer or a bleeding ulcer until proved other. (I trust you're not an amateur vegetarian.) Get that worked up for real.
