Unclear: An ultrasound is one of the best tests i medicine when it is used to diagnose gallstones. However, that does not mean you need your gallbladder removed. A gallbladder should be removed for symptoms - stones are not an indication  for surgery.

No not necessarily: Get a Hida or desida scan before surgery.  May help to determine if gallstones are symptomatic or just incidental findings.

Typical report: Gallstones can vary in size from very small to quite large. Normal GB wall is 2 to 3 mm thick, 5mm is not much above normal. Pericholecystic  fluid is typically seen on US and suggests acute inflammation.

Although larger one are usually asymotomatc 
Because they can’t pass thru to the biliary duct.

Gallstones: It depends on your symptoms and any degree of obstruction whether you have it taken out  definitely start watching your diet to prevent unnecessary stimulation.

If you have: no symptoms  your both  conditions could be observed . GB polyps has malignant potential but usually they are 2 cm or more in size . If over the counter  pain medications are not helping you to relive pain , your best option after colonoscopy is removal of BG , which is relatively low risk surgery.
