Cause of tear....: This is difficult to answer without knowing the time frame of when the injury occurred and when the various types of  treatments were tried as well as the severity and location of the tear. Ill timed p.t. and steroid use can not only  be unsuccessful, but, in some specific circumstances, can actually delay the healing. It might be time for another opinion.

Hard to say: Multiple potential causes.  It would be in part related to the condition that you had surgery for, or could be related to the surgery itself, or it might have occurred with physical therapy.  I certainly would not hesitate to discuss with your surgeon.

Surgery: The retinaculum is a band that holds the peroneus muscle tendons down against the outside aspect of the foot. It provides a tunnel through which the tendons maintain their position and fulcrum action.

Surgery is the only way to correct this problem. Since the retinaculum is ligamentous and has no true blood supply it cannot heal itself and thus surgical repair is necessary.

May or may not : Be related. The peroneal longus tendon inserts on the base of the first metatarsal and this may have something to do with the function of the great toe. Xrays to rule out hallux limitus/rigidus and clinical exam are warranted.

Depends: There is a spectrum of disease when it comes to achilles tendinitis/tendinosis. An MRI can identify if it is achilles' tendon disease vs. Inflammation of the lining around the tendon. Rest and physical therapy usually ameliorates the problem, but sometimes operative treatment with debridement and even tendon transfer is required.
