Best is what works: The best therapy is what works.  That being said, it is generally preferred that we utilize non-invasive treatments (physical therapy, medications, etc.) before we utilize invasive treatments (injections, surgery, etc.).

Short or long term?: In the short-term, injections can achieve faster relief if the nerves are being compressed in specific locations.  However, longer-term studies have shown that within 3-6 months outcomes are similar.  Keep in mind, most people will actually improve without specific therapy given enough time - although stretching, core strengthening, weight loss, and not smoking help a great deal.

Sciatica: Firstly an evaluation and probably an MRI. Then possibly PT, with Medrol (methylprednisolone) Dose pak, Calcium channel modulators (gabapentin, pregabalin, enacarbil gabapentin), NSAID's,  epidural steroid injection,  and possibly PRP injections.

No: If the pain is tolerable at present and you have no fever, weakness of the knee, ankle or foot, or loss of bowel or bladder control, you can probably wait a day or so. Numbness or tingling are okay, but not true weakness (as opposed to early give-way due to pain). The nurse probably doesn't want to get sued for telling you NOT to go to the ER if it's something "bad."

Sciatica: It is very likely you have sciatica.  The sciatic nerve can be irritated with carrying weight, excessive bending or a simple back strain. Furthermore the relative weakness of your ab muscles can contribute.  Recommend nsaids, exercise and ab strengthening.  Might want to check with a pt for help as well.

Otherwise, your doc can give a prescription for formal pt instruction (big help for most).
