Probably not: At your age a heart attack is generally unlikely especially if you have another explanation for your symptoms (pinched nerve) but either way you should see your doctor to sort it out and maybe a neurologist if it turns out to be nerve pain.

Yes: A nerve root compression in your neck (cervical spine) can do this - at your age, a ruptured disc would be the most likely culprit, usually trauma related. Tumors are thankfully rare but in the differential diagnosis. An MRI and EMG would confirm this.

Not likely to be: cardiac in etiology. Ask for a referral to a neurosurgeon or orthopedist for treatment of your neck.

Go to ER: Given your age, the odds of a heart attack are low, but you do have risk factors with the blood pressure and smoking, and none of us over the internet can diagnose you.  When it is such a potential life threatening matter, you should seek medical attention immediately.

Only A or B?: I've never heard of a mini heart attack, but I expect your heart may not be as healthy as some 22 yr olds'. A cervical disc problem may be found, but I'm not happy trying to select A or B. You are heading down a bad highway and the pains and distress that you'll be heading toward make this look like a walk in the park. Please do comsider making changes. Talk to older folk about it life.

: Disc bulges&amp;herniations in the thoracic spine could well be a cause of chest wall pain but usually by impinging on nerve roots exiting the upper thoracic spine. I would ask your neurologist or neurosurgeon if any of your disc problems are pressing on the spinal cord itself &amp; are a potential cause of chest pains different from the radicular pains which are usually sharp &amp; provoked by a deep breath.

Risk factors...: Risk factors for heart attack include: smoking, uncontrolled hypertension, uncontrolled cholesterol, sedentary lifestyle. Males and post-menopausal females are at higher risk as well as family history of a heart attack at a younger age. Diabetes was always thought of as a risk factor but new evidence suggests that well-controlled diabetes is probably not a risk factor.
