Prinzmetal angina is caused my spasm (pinching) of larger heart arteries.

These processes of ventricular alteration would almost always also affect the t wave. Some processes of electrolyte or ischemic nature can change the t wave without changing qrs.

Yes: It happens.  Ventricular or atrial tachycardia is the name not to mention atrial fib.  Get a cardiogram and an echo if you have any symptoms.

A cruel choice: But honesty, delivered gently and with compassion would be my advice. Shielding people "for their own good" seems paternalistic and disrespectful to me. I would want to know where my son was and what happened.
