Uterine fibroids ?: At 65, uterine fibroids are usually diminishing in size.  If there is "uterine fluid" found by ultrasound, it is always better to have a tissue diagnosis, as this may be indicative of a process which requires more definitive treatment.

In menopause: the uterine lining and fibroids should be inactive. These findings could represent post menopausal estrogen production/stimulation, from conversion of androgens to estriol, by peripheral aromatase conversion. Discuss with your doctor, as medications and/or endometrial sampling may be appropriate.

i have right achey pain from below my ribs to my  upper back and nauseous on and off since november . saw gyn and had an ultrasound .. negative for ovarian cysts, fibroids , iud misplacment and endometriosis. pcp didn't think it waw my gallbladder.

Gallbladder: Hi, considering the location of pain it sounds more like gallbladder or liver issue. You should have an ultrasound gallbladder related. And have your liver enzymes checked, and your bilirubin levels checked.

Abnormal uterus: Dub is dysfunctional uterine bleeding and should be used to describe abnormal bleeding related to menstrual or hormonal issues. 
Heterogenous when used to describe the uterus, usually means abnormalities in the wall of the uterus, commonly fibroids or adenomyosis. You should get a better explanation as it relates to you from your gynecologist.

Fibroid growth: Growth of fibroids prior to menopause is common and not usually concerning. Treatment for fibroids is based on symptoms (pain, bleeding, pressure, etc) and not out of a concern for cancer which they rarely are. Estrogen causes fibroids to grow. Treatments can include medical treatments to lower or block estrogen levels or surgery to remove the fibroids or the uterus and fibroids.

i had an ultrasound performed which found a fibroid, but i also have what looks something like a black hole, what could it be?

Cyst, pregnancy: If something looks like a black hole on pelvic ultrasound, more information is needed to say what it is.  If you have a positive pregnancy test, it could be an early gestational sac or ectopic pregnancy.  Otherwise other possibilities include ovarian cysts.  You should follow-up with your physician about the results of this ultrasound.

can fibroids be missed on an ultrasound?

Yes, but: Fibroids can be missed, but only in certain circumstances. If the fibroids are very small or non-distinct, then they may be difficult to spot on an ultrasound. Or if a fibroid is pedunculated and high up above the uterus then it may be hard to see on a transvaginal scan. In those cases a trans-abdominal scan would be needed. But in general, ultrasound is the best way to see fibroids.
