Endometriosis: It sounds to me that you are trying to find out how endometriosis is diagnosed and treated. You are right that there are no good imaging studies for this and the best diagnosis is made by (1) high index of suspicion--pain is cyclic around menses (2) laparoscopic surgery for diagnosis and treatment. A long history of endometriosis also leads to "adhesions" that can only be treated surgically.

Para-ovarian cysts: Either ovarian cysts or para-ovarian cysts will release fluid when ruptured, and one of either would produce "fluid behind the uterus", which is harmless. You may have experienced sharp pain just before getting the ultrasound which showed "fluid", which may have been the ruptured cyst which produced the liquid.

i just found out that i have 3.62.4 cm cystic lesion on the left adnexa is stable and a 1.4 cm calcification un the left adnexa the uterus us gone there is no fluid in the pelvis area what does that all mean im type 2 also should i be worried?

Possible answers: Its most likely a stable ovarian cystic tumor (benign), though calcifications can be associated with teratomas (also benign).  Teratomas do tend to get bigger.  The calcification could also be to past surgery.  I'm assuming a CA-125 was done and probably an MRI would have more luck defining this cyst if really concerned.

No: But a cyst this size should be dealt with surgically/laparoscopically.

Possible: Discuss with your GYN regarding possible side effects of surgery.

Possibly: But could be due to many other causes. Would need a full detailed history and exam as well as possibly some imaging studies. Go back and see the surgeon who removed the cyst and have yourself evaluated. Good luck.

Yes: Pelvic examination can evaluate the ovaries and sometimes tell if they are enlarged. However, ultrasound is more accurate.  (but remember there are always small cysts on the ovaries.  The egg comes from a cyst with ovulation.).

Yes: cysts are common in the ovaries and usually don't affect fertility. If you are having significant pain, unusual bleeding, or missed periods, see a doctor

Not dangerous, but.: it is not dangerous, but first it is large , and can cause a lot of pain.
second complex cyst are not all benign, it needs to be classified by biopsy.
they can do that by laproscopy.
