Thyroid deficiency: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, elevated TSH and low Ft4 indicate hypothyroidism and your doctor should prescribe thyroid medication.

tsh historically 4.5-4.7. recent tsh was 6.02 uiu/ml. primary care md repeated the tsh and added t4 free. results = tsh 3.56 and t4 0.96. i have severe fatigue, muscle aches, sensitivity to cold + some hair loss. could this be hypothyroidism?

Confusing: TSH and Free T4 are two different tests.  A suppressed TSH is seen when the FT4 is too high, either due to hyperthyroidism or from taking too much levothyroxine.  the FT4 should be high, although these changes can be subtle.

No need to worry: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Having said that, your results are close enough to normal that you do not need to be concerned. You may wish to repeat the test in a year or so to see the trend, especially for TSH.

Need Eval: Goal TSH for people with hypothyroidism is 0.3 to 3 IU/mL your T4 is mid nl &amp; free T3 on the low end of nl. It is unlikely, the fevers are from the thyroid. You should repeat them, but take your Synthroid (thyroxine) after the labs that day. Also, when ill the TSH &amp; free T3 can decrease, something called euthyroid sick syndrome &amp; gen wouldn't check while ill. The cause of fevers also cause the abnormal labs.

Depends on details: Lab values can vary (even if there has been no change in a patient's status nor medications). That is why lab results are "just numbers," and one should see her doctor (who knows the whole story) for an interpretation of her results. A "reflex" test is done usually if the first test is outside the reference range. Thus, if TSH is above or below the reference range, a Free T4 hormone test is done.

Mild hypothyroidism: The elevated TSH means that your pituitary gland is not happy with your thyroid level, and would like to see it higher despite the "normal" free T4 level. If you do not have any symptoms of hypothyroidism, we call this "subclinical hypothyroidism". Treatment may not be needed unless you develop symptoms or the TSH goes progressively higher. You should not start treatment based on one abnormal test
