i have lupus anticoagulant and dr. is wanting to put me on xorelto! i've read mixed reviews mostly against, & cost is a factor? what is your opinion?

Use caution: Xarelto is awesome if you listen to the manufacturer. Unlike coumadin, (warfarin) you do not require regular blood draws to evaluate effect. However, there is also no clear reversal agent if one does sustain a bleed. It can be a terrible situation when someone has a brain hemorrhage, etc, from relatively trivial fall.

See below: All laboratory results need to be interpreted in the clinical context and the doctor who ordered the tests is usually in the best position to do that. Talk to the doctor who ordered the tests. Having said that, if your hemoglobin is 10.2, you are anemic, probably due to iron deficiency. Please discuss it with your doctor who knows more about your health.
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

Usually negative: Anti-nuclear antibodies are almost always positive if it's really lupus. These tests are fraught with technical pitfalls, and are never more than adjuncts to a clinical diagnosis.

ANA, Sm, antidsDNA: Laboratory tests are important but not the only factor in diagnosing lupus.  Generally the screening test is for anti-nuclear antibodies (ANA) more specific tests are antibodies to sm antigen and double stranded dna.

i had a 1:160 ana blood result. further testing for lupus was negative and my rheumatoid factor was normal. what could the high ana be caused by?

Depends: For many labs 1:160 is the threshold to report the test as positive. All lab results need to be interpreted in the clinical context. ANA has a large incidence of false positive results. See this site for more info and discuss the issue with your doctor.

In several ways!: Lupus can be associated with dry-eye syndrome, typically more severe in sjogren's syndrome.  Very severe lupus can causes optic neuritis, but this is quite rare.  The most common problem with the eye in lupus patients is the development of cataracts from treatment with steroids, one of many reasons doses should be kept low. Plaquenyl eye toxicity is nill, by using less than 6 mg/kg of body weight.

No: Positive is positive. However, it is the degree of positivity that is clinically important. Low positive values are generally not clinically important.

had a lupus anticoagulant test done all was normal except for a high thrombin time at 22.8 is this serious? factor 5 also runs in my family.

Accepted Answer

Normally:  a factor V  deficiency can cause a prolonged thrombin time But your bleeding time and platelet count should be unaffected. Certain medications and other diseases could also cause elevated thrombin time.
