Post op cataract: It appears from your question that the surgery was done October 14th (today is 10/17). There are a number of visual symptoms that appear in the first few days / weeks after cataract surgery that resolve spontaneously. The temporal crescent and flickering are very common and almost always resolve. One theory is that the implant becomes more secured within the capsular bag during this period of time

It is possible: Some of the newer toric lens implants have a light yellow tint which is designed to reduce glare.  If the other eye has a clear lens implant some patients will notice a small difference between the two eyes.  If the other eye has an early cataract which is also slightly yellow, or a matching implant, few  people can see a difference. Most will adapt and not notice after a few weeks.

Probably not: Repositioning a toric iol  is easily done in the early postoperative period. However 2 years later entails significant risk due to adhesions that have formed between the iol and the lens capsule. An examination to determine if the axis of your corneal astigmatism aligns with the astigmatic axis of the iol  needs to be performed first to diagnose the reason for your poor result before any treatment.

i had cataract surgery in both eyes a year ago. i am left with a terrible astigmatism in my left eye. the right eye is fine. can the toric lens he used be removed and a standard lens be put in?

Yes: The options can include a toric hard contact lens vs. Glasses vs. Removal of the toric lens and placement of another intraocular lens.  A second surgery is much more difficult and has a higher complication rate.  Depending on the situation, you may need to get a vitrectomy surgery in addition and you my need a rigid contact lens regardless if there is significant corneal abnormalities.

Good option: For those with moderate to significant astigmatism, which is a good portion of the population, choosing a toric lens implant for your cataract surgery will reduce your dependence on glasses post surgery. I advise my patients who have astigmatism to opt for a toric lens if they can afford the premium cost. If they can't, glasses will correct the astigmatism post surgery.

Individual preferenc: Lense manufactureres are in constant battle to grab a better share of this large and lucrative market.  With minor changes in plastics, curvature and preservation - each new lense is advertised as the best out there.  Fit, vision and comfort are individualized and each type of lens may be ideal for some, not so good for others.  The same goes for acuvue oasys v. Sofmed.  Try each.

could it be absolutely neccesary to wear toric lenses for astigmatism?

Accepted Answer

No: If you do not correct the astigmatism with toric lenses, your vision will not be as sharp, but in some cases, is still acceptable.  Have your practitioner give a pair of the appropriate spherical lenses as part of the contact lens fitting process.
