High spot: Since you had a recent filling , your filling may be a little high. If the bite is off, the tooth can become sensitive. Hitting heavy on filling can bruise the tooth socket and lead to sharp pain on chewing. Go back to dentist and have bite adjusted. Very common problem.

: Sight unseen, I suspect it is the tooth with the filling. It may be "leaking" and the filling probably needs to be replaced, especially if it is 10+ years old. Radiographs don't always show every problem, so you need sometimes treat from experience.

Malocclusion: Malocclusion will cause unconcious tooth grinding and bruxing at night, leading to acute pain and possible tooth mobility in the morning.  Was the gum bleeding or abscess.  Pulpal involvement occurring with prolonged bruxing/grinding will also cause irreversible pulpitis, pulpal necrosis, abscess, and ultimately pain.  Check with your dentist.

Return to your Dr.: Without seeing the "tooth shell", it is hard to know what that is, but it sounds like you already had a dentist who removed the tooth for you. Sometimes, a small fragment of separated bone can come out to the surface on its own, or it could be something your dentist placed for tissue healing but dislodged. If you return to the doctor who saw you for extraction, you will quickly find out.
