Preserve the molar: If your molar is the first molar, you should try to keep it since it is the most important tooth in your dentition in term of posterior support, chewing function, and effecting the esthetic and longevity of the front teeth.  Any other molar and financial hardship is a concern, extract them.

Tooth pain: Chronic tooth pain can be due to bruxism (chronic teeth grinding and/or clenching), infection of the tooth or gums, a fractured tooth (very common under large silver fillings).
Bruxism can cause TMJ dysfunction with pain referred to the ear.
A definitive diagnosis cannot be made without a clinical exam, x-rays, and additional information.
Check again with your dentist, and an ENT for ear pain.

extreme lower tooth pain, 3rd tooth to left of 1st right molar. if i have an abscess, or something else can they do a root canal on front teeth?

See a dentist ASAP: If you have a toothache, you may have a cavity, wisdom teeth or advanced gum disease. A tooth pain won't go away without treatment and the infection may spread to your jaw, cheeks, sinus and to other areas of your head and neck. You can use NSAID OTC or Tylenol (acetaminophen) for pain control. See a dentist ASAP for assessment.

upper molar crowned, root canalled(same tooth), 3 roots done by x-ray shown, still mild tooth pain always, does this mean 4th? canal?

Quite likely.: Quite likely. The first upper molars often have a fourth canal commonly called the mb 2. (mesial buccal). Many endodontic specialists use high powered microscopes to detect these very small canals. My suggestion would be to have an evaluation by an endodontist that uses this technology.

chipped back molar no tooth pain but tongue sore from rubbing. discomfort in upper and lower jaw area. molar on bottom . can this cause jaw pain 2?

Chipped tooth: Your jaw is more likely to be sore from bruxism (clenching\grinding of teeth) or from biting down on something hard that caused the tooth fracture in the first place. Pain can also radiate from the chipped tooth, an adjacent tooth or the sore tongue. Regardless, see your own dentist asap for a thorough evaluation and appropriate treatment.

Fistula....: I personally advise you first see a dentist to discuss your options based upon a clinical exam and x-rays. You should understand the pros and cons of each option and prognosis of Endodontic treatment followed by restorative treatment. Your General Dentist may then elect to treat you or refer you to an Endodontist (or Oral Surgeon).

a week ago i received a filling of a lower molar which had a 50/50 shot of needing a root canal.    i have not had any pain; however, over the past two days my throat has been bothering me.  should i be worried about infection even without any tooth pain?

i get sinus pain, shooting left ear pain + tooth pain on the left side lower molar+ canker sores on my lip. how are lip sores related to sinus pain?

Unrelated problems: 1. A sinus infection or inflammation can cause a ear pain, These problems should be managed by your PCP or ENT. 2. Toothache -see a dentist  3. Mouth sores (aphthous ulcers) are painful sores inside the mouth. The sore may be painful for three or four days, but complete healing usually takes ten to fifteen days. Canker sores may recur many times each year. No cure is available.
