Good concern: It is good that you are already aware of such post-extraction complications. If you were given a post-op instruction, please read well and follow it. Make sure not to spit hard if you rinse your mouth (keep it to minimum for the first couple days), stay on a soft diet, if unsure request a post-extraction exam or check to make sure healing is normal. Losing blood clot leads to dry socket.

Three weeks, no: The socket should be closed by now. Once the gum tissue fills in the void, there will be no dry socket.

Yes: A dry socket is caused by breakdown of the  blood clot in the tooth socket. This usually occures 3 to 7 days after the extraction. Your dentist can place a dressing in the extraction site to mimimize the discomfort.

Yes: Almost a week (often said "5 days") is a typical risk period for dry socket. If your socket is large and open, and there is not much clotted blood, you are more likely to have dry socket. Smokers have higher chance of getting it also.

A hole: Dry socket is essentially when a blood clot either doesn't form or is prematurely broken down or lost from the extraction site...Leaving an open wound.

Probably: A dry socket usually occurs in the first few days. After that, the epithelium closes over the socket, basically preventing exposed bone.

Unlikely: Most of the healing has taken place.  If the blood clot is intact during the healing process then chances are the socket is healing.  The chance of developing dry socket is greatly diminished.

Dry socket: This refers to after a dental extraction the clot is prematurely removed or comes out .  The socket and nerve stump is exposed to the environment and is usually painful

Loss of clot & : Failure of new clot to form, release of chemical pain mediators.  Normal to feel some pain & discomfort after, however...If on 3-4th day after the procedure the pain increases substantially & character of pain changes (throbbing more, radiating more to ear & temple) & if the pain medicine that you were taking is now grossly inadequate, then fibrinolytic alveolitis is high on list of possibilities.

Increased risk: Anything that can cause the blood clot to pull out of the socket can cause a dry socket. Any kind of negative pressure - vacuum- will do so. That's why smoking, spitting and drinking through a straw are to be avoided for at least 48 hours. Good Luck.
