Stressed Bone Fx.: OOO! could be a stress fracture of the 5th metatarsal. You might want to take a trip to the local surgery and have an x-ray to see if the metatarsal is sick. There are other possibilities as well, but my money is on the bone.

The screw: should be removed.... 6 months is sufficient time for solid healing.....go back to the surgeon who did the original surgery!

Pain : Is not normal. Need an x-ray to make sure no fracture is present si suggest podiatrist in area. Rest, ice, elevation and compression should be used until its checked. Could be simple contusion but best to have it checked.

I would: See a foot doctor. The question is are the lumps or bumps really on the tendon or in the subcutaneous tissues.....either way you should be examined,  much easier to make a diagnosis like that in person...
Possible ganglionic cyst is an example without seeing you but if I saw your foot inperson I would know right away if that's what you had.

Circulation issue: Do you smoke?  Have you ever noticed any discoloration  in your toes?  Sounds like a circulation issue, though could be other reasons for this to happen.  Regardless of the cause, because this could eventually affect other toes, you should have this checked out.
