Many things may: Try to wear shoes with plenty of room.. If pain persists, an x-ray would be indicated. Possibly lab work as well.

Trauma: Even microtrauma from an ill fitting shoe can be a possibility. Swelling of a toe can cause throbbing. Is there any abnormality of the toe? Anything on the skin like a corn? Are there any cracks on the underside? Is it red? Or is it totally normal in appearance and throbs? If so, it might be neuropathy....A stress fracture could be a possibility as well. A doctor may want to x-ray to see the bone.
