puffiness/redness/burning sensation is back after big toenail removal on 3/25/15. will i most likely have to have the toenail removal procedure again?

TO KNIFE A NAIL BED: Sounds like a repeat procedure may be coming, especially if there is some toe nail left.  But in situations like these, patients have best results  when doing warm water soak 3 times/day, otherwise ensure area is clean &amp; dry, give it plenty of exposure to air, &amp; daily bandage changes (youtube instructions on best bandaging, almost all videos with over 50k views are reliable)

my child has been on dicloxacillin for 5 days for redness around her toenail (no ingrown).  the pain is gone but no change in redness.  normal?

Redness should go: It sounds like infection is improving- but at 5dys the redness should also be a little better. But if infection was deep   then no worsening redness may be a good sign- i would contact doctor- b/c if is a MRSA infection the Dicloxacillin may not work completely- but another simple to use antibiotic will treat it. Meds such as Bactrim (sulfamethoxazole and trimethoprim) and Doxycycline depending on age and allergies of your child.

It can be: Most of the time the skin around the nail will become red & swollen and will have pain when the skin or nail is touched.

Without: Seeing it it could be many things but my guess would be an ingrown toenail which is inflamed by activity. See your podiatrist for treatment . Hope this helps.

It's : It's hard to say without examining you whether you actually have an infection, an abscess, or just an overgrowth of granulation tissue from the ingrown nail.  If your primary care doctor is unable to help you, you may want to contact a podiatrist in your area.  If needed, they can cut out the piece of the nail that is ingrown - sometimes at the root of the nail - and apply a medication to stop the nail from re-growing in that spot and becoming ingrown again.

In the meantime, make sure you are soaking your toe in warm water 2-3 times a day for 20 minutes at a time.  If it is worsening instead of improving, or if you have increased redness, swelling, pain, drainage, or fever, call your doctor immediately.

Couple thoughts: You may have an infection that requires medical treatment but give this a try:
soak toe in diluted Acetic Acid (vinegar mixed about 1:20 with water) for about 20 minutes.  Then, take a piece of cotton ball or q tip and jam it down the corner where the nail is trying to grow inward.  This will make a space between the skin and the nail so that the nail will grow upward.  Nails grow only 1 mm/ month.

Circulation: Underneath your toenails are bllodvvessels. Red blood cells when oxygen is removed from them turn purple and therefore you get purple discoloration in your feet. Your feet hang down and we call this acrocyanosis. It's part of the normal physiology.

Try-: You might have to have that area of the toenail trimmed off. If the toe gets real sore and red and some pus then an antibiotic might be needed. Local treatment to try are Epsom Salt soaks, Neosporin Ointment 3x/d and hydrocortisone cream 2x/d.

Possible infection: Pain and redness indicate likely infection unless he was injured.
Infection requires antibiotic treatment promptly.
So take him to see a doctor right away and seek medical consultation for treatment of his infection. Also check his temperature as most often infection is accompanies by fever.
