COPD: The rattling is usually fluid which can be from asthma or emphesema- two types of copd.  Don't smoke- this paralyzes the small hairs in the lungs which prevents secretions from being clears.  Allergies with post nasal drip can also lead to noises.

Arrhythmia: See your doc.  You are having palpitations, which may indicate an irregular heart beat.  There are many types, most of which are not serious.  Testing, such as an ECG, overnight monitor, and blood work will likely be helpful in tracking this down.  In the meantime. decrease or eliminate caffeine, and avoid stress.

Possible anxiety : This can be anxiety or panic attacks. Try to do deep breathing exercises,  meditation, regular exercises, talk to aomebody close to you if you notice it coming on , this can help calm you down. If these fail, you may need to take oral medication that can help as needed or even daily meds to prevent subsequent attacks talk to your dr about this. Good luck.

The same: I do not believe there is any significant difference in the side effect profile.  Could this be related to the pill / capsule getting stuck in the throat?  Maybe take with food?

Could have breathed: Could have breathed in some chemicals that caused irritation.

can an esophigal  spasm feel like a pvc i had a quick sensation in chest throat and it hurt it was over in a second but no thud like pvc?

PVC: An esophageal spasm can feel like a pvc, although in my experience the symptom of an  esophageal spam tends to last more than just a second and often is assoc w reflux. Pvc sensations can be highly variable ranging from no symptoms at all to a kick, chest pain and shortness of breath. Continued symptoms should be evaluated as both conditions can have long term consequences.
