Headache with neck: Head ache with tight neck of acute onset with fever need to rule out meningeal irritation like happens in meningitis. If it is a chronic problem for months may be related to neck arthritis, neural compression, have it checked out. If work up negative, may be fibromyalgia.

Electrolytes v Other: Get blood work if you haven't already to include blood levels, kidney and liver function plus sodium, calcium and others like folate (folic acid) and vitamin B12. I see you had thyroid function checked. With nausea and unusual food cravings, if these tests are all negative, then a head CT may be considered to rule out organic conditions in the brain before addressing potential emotional causes like anxiety.

Not likely: Full-blown serotonin syndrome is a medical emergency, and can include loss of consciousness and seizures. Sounds like the cough syrup with dxm is being abused, and your doctors, who are trying to treat anxiety and depression, are flying "blind" if they don't know about your drug use. Be honest with yourself and with them, and let them know what is going on. Stop the cough syrup, talk to your docs.

i would seek medical help and obtain an MRI C-spine, face and brain to see any anatomical causes

Need to see MD: The list of potential causes for your symptoms is very large.  They could all be caused by anxiety, but there is no way to know this without a careful history and physical examination.  One needs to know the duration of each of these symptoms and whether they vary in severity and duration.
