my toes feel tight and look a very little bit swollen ish. its hard to tell its not swollen to a point where its noticeable. i notice this when im standing for a long time for about an hour or so. is this normal? once i sit it feels normal again?

Seek medical advice : Your description of your toes is consistent with swelling to your digits       standing and sitting are pretty much the same in  controlling swelling.      in other words this does not control swelling.   because of your age I would seek medical attention this is not common at your age group

3 toes losing : Sensation is something to be concerned about and you should see a doctor for evaluation. Any low back issues/pain? Any medical history? 
Without seeing a picture of your skin issue, I can offer that it may be herniated fat which is not an issue.

tried a new podiatrist. said my orthotics were old and made new ones, gave me temporaries to use in the interim.  tried the temporaries now toes tight?

Contact podiatrist: I'd get in touch with your podiatrist and see about getting some new temporaries.  Hopefully the new permanent ones will have a better fit than the temporaries, but if not, just have your podiatrist make some adjustments or make new ones.

Hematoma..: It sound like you have managed to traumatize the bottom of your toe causing a hematoma. It is probably best to have the toe evaluated to any possible problems.

: I assume you mean the toenail is brown and flaky. You could be dealing with a fungal/bacterial infection of the toenail. Topical medicine can help oral medication also available. See your family doctor or podiatrist for diagnosis and treatment
