my blood sugar levels are 200 measured 2 hours after food. i have been advised medication by my pcp. i have drastically cut down on carbs in the last 3 days. i still feel fatigued and thirsty all the time. 
is it related to food intake?

: In type 2 Diabetes your body either resists the effects of insulin, a hormone that regulates the movement of sugar into your cells, or doesn't produce enough insulin to maintain normal glucose levels. Steady, long term weight loss is the only way to reverse this disease. You should be monitoring your glucose levels daily to avoid hypoglycemia and take the prescribed medication.

how do i overcome the guilt after eating a meal high in carbohydrates after being on a low carb eating regime and how do i manage my cravings?

See below: Having excessive emotional attachments to food may cause unnecessary guilt. You may need to eat more carbs if you are underweight or engage in moderate to heavy exercise on a regular basis. Consult with your doctor for any medical issues and to assess your wgt and BMI.  If you suffer from eating disorders, see a therapist. Eating healthy small portions through the day may help reduce cravings.

will metformin drop high blood sugar levels if it's taken after eating many carbs?

No: To my understanding metformin does not decrease blood sugar.. It just does not allow it to go as high it is without metformin.

Ok: Probably due to the high intake of simple carbs. Try high protiene breakfast to avoid the post carb crash.

some people experience palps after eating heavy meals that are rich in carb, sugar, fat, high level of msg, nitrates & sodium (webmd.com). why could?

Multiple responses: Carbs and sugar effect changes in insulin and blood sugar levels which, when abnormal, can cause palpitations. SMG and sodium causes salt and fluid retention, high blood pressure and heart failure with palpitations.  Nitrates cause asthma and allergies in some.  I'm not aware of fat causing immediate palpitations.

took bp before eating high carb meal 127/85 pulse 80, then stood for 12mins. bp was 108/85 pulse 146 does this suggest postprandial hypotension?

Rapid heart rhythm: Your b p dropped significantly after standing but the pulse rate of 146 was very rapid. I wonder if this was accurate. We're you lightheaded when your b p dropped? If not, then it is not problematic. A heart rate of 146 would be problematic. Double check your pulse to be sure and see how long it takes to return to normal. Discussion with your physician is recommended.

Many possible reason: If you eat salty foods or high carbohydrate foods you can become thirst from a change in serum osmolaltiy or rise in your blood sugar.

Salty: foods like pizza often cause thirst because of a high salt content. Persistent thirstiness can be seen in dehydration, diabetes, intake of very dry foods. Since you're not always thirsty and apparently feel well, I doubt you have any serious medical issue.

Hypoglycemia: It is possible that you get a rush of Insulin after you eat cars which leads to a drop in your sugar.  It is best to eat complex carbs not plain sugar and to eat brown rice, brown pasta etc.
