i went from having incredibly heavy periods for several months to spotting instead of a period for 13 days. ultrasound showed thick endometrial stripe?

Blood test : Rule out pregnancy, If negative LH, FSH, estradiol, progesterone, prolactin and thyroid blood testing  to see if a hormonal problem is the culprit. A sonohysterogram, endometrial biopsy and a hysteroscopy to  evaluate what is in the uterus may be needed. If biopsy is negative and no polyp, provera (medroxyprogesterone)  is given to bleed the thick lining. If hormone off birth control can be used to regularized cycle.

i have no period right now.i had a tvs ultrasound today.the doctor said normal sized uterus, thick endometrial stripe, normal ovaries and cervix.

Congratulations ! ?: Your transvaginal sonogram seems to be normal...depending on your age, parity, personal and family history , the medical indication for the tvs etc.
If by "I have no period now" you mean you are postmenopausal or if you use estrogens, you may need a saline sonohysterogram or further evaluation by endometrial biopsy. Consult the gynecologist who knows you.

is a thick 18mm endometrial stripe a cause of concern? i had an us 2 days before my period. but looked at an us from 2 years ago and endo was 1.1cm.

The endometrial: stripe is thickest right before a period and thinnest at the end of the period.  18 mm is slightly above the maximum considered normal.  This is called "endometrial hyperplasia."  Its significance depends partly on your age, menstrual pattern &amp; the degree of irregularity of the stripe.  Your Gyno may need to "sample" or biopsy the endometrium to get a firm diagnosis. Poly cystic ovaries: Your uterus is normal and bends forward rather than backward. The lining of your uterus is thick from hormonal imbalance. Your ovaries have lots of cysts in them also from hormonal imbalance. This is from Insulin resistance which is a pre-diabetic condition. It is called poly cystic ovary syndrome (pcos).

why would part of my endometrial stripe be thin and part be thick (over 1cm)?  this is 7 days after end of a abnormal 21 day period.

Uneven "lawn mowing": When the estrogen/progesterone sequencing is "out of phase" for whatever reason then the endometrium will be "disordered" just like a drunken landscaper mowing your yard or a tornado that has plowed through a corn field. Thus when the period starts only the part of the endometrium that is "tallest" will shed leaving the rest in place to begin to bleed again later.

Before/after menopau: After menopause, the stripe should be &lt; 5 mm.  Before menopause, it can be thickenr.
