Get kidneys checked: I would be cautious. Do not say what caused your rhabdomyolysis last year but if you're having pain after taking something I would stop taking it.  I would seek the advice of your doctor may want to check some lab tests to determine your kidneys are functioning correctly

Stop the Creatine!: Taking creatine for muscles is popular but potentially dangerous. Creatine can cause permanent damage to your kidneys even if you stop the creatine. So, it's better to NOT take creatine in the first place. You have diabetes, which can affect the kidneys, so creatine is even more dangerous for you. Drinking lots of water is best way to replenish electrolytes before and after weight training.

the lack of such enzyme could cause problem like hemolysis in the blood 
creatine is not one of the product that will lead to a problem
it is safe to take but the same time people who use this product have to monitor their liver function and the kidney function

Needs workup: A creatinine of 1.5 could mean you have stage III chronic kidney disease.  It is important to know if the creatinine is changing and how fast.  Stage III chronic kidney disease can be associated with diabetes, hypertension and other diseases.  You need to be worked up with blood, urine and ultrasound.  It could be serious.

Taking with other drugs: tylenol, (acetaminophen) caffeine, ephedra may increase side effect.
