Infertility: The ESR is not in any way diagnostic of any specific medical condition.  There are at least a dozen physical and chemical processes involved in getting pregnant. All of them need to be tested by an infertility specialist before determining what is causing your inability to conceive.

i am 51 days pregnant. last week i took  blood test. level was 502. now yesterday's test reported 427. no bleeding , will this affect my pregnancy?

HCG: Your HCG level should be rising, not declining.  Hcg (human chorionic gonadotropin) should approximately double every 48 hours in early pregnancy.  I would contact the physician ordering the blood work and ask about these results.  Best wishes!

Microgynon?: Urine or blood pregnancy tests are very accurate, regardless of whether a woman is on birth control pills or not. One can do the test about 3 weeks after sex (3 weeks after when conception could have occurred). The result almost always will be negative, if one is using her birth control pills properly. Rarely is a pregnancy test an urgent matter.

Yes, and depends: Alpha feto protein in maternal serum can be quantitated at any time, during pregnancy.  Why is the test being done at 21 weeks?  AFP measurements are done for two main reasons; (1) screening test for fetal neural tube defects (2) screening for liver cancer.  Levels during pregnancy vary with the time of gestation.

Ultrasound to: Confirm pregnancy.  Depending on how far along the pregnancy is, you may or may not see a baby per se, but ultrasound will confirm pregnancy by showing the gestational sack and related structures. The blood test is nearly 100% accurate.
