Please : Please reply with more information. The number you gave is not a ratio. It is very difficult to interpret what a random urine creatinine level means without knowing the microalbumin level and looking at the ratio. Lastly, whoever ordered this test should be able to explain why they ordered it and what the results mean. Please reply with more information or follow up with your doctor.

Proteinuria: You have proteinuria (p) and the first thing is to have a 24 hour urine collection to see the amount of protein in your urine and to assess your kidney function. Have a renal ultrasound to assess kidneys' size
depending on the p, i would send off blood for aso, anca 2, complement C3 and c4, hiv, hepatitis b antibody to see if a cause could be found for the p, if not a kidney biopsy may be needed.

i have symptoms of muscle cramping and pain all over my body. i had a random urine test done and my albumin levels were just over 300 mg/l and my creatinine levels are 600 mg/dl. is this macroalbuminuria and would it be causing my symptoms?

: If the numbers, especially the units, you gave are accurate, you only have mild microalbuminuria (50 mg/g) and that has nothing to do with pain and muscle cramp. However, a systemic disease with those symptoms, e.g. statin induced rhabdomyolysis, can cause mild microalbuminuria. You need more tests than just this urine result for the definite diagnosis.

Several things: The most likely is inadequate collection of urine such that some of the 24 hour sample was discarded. Other causes are low muscle mass, which can occur in very small stature people, advanced age, bed bound individuals, and in patients with cirrhosis.

The : The most common reason for low creatinine is that you drink a lot of fluids. It is not abnormal to have low creatinine, it's just that the people doing the lab test want the urine concentrated enough that it can pick up trace amounts of the drugs they are testing for. Dilute urine= less of drug in urine sample. Some people have a medical condition that makes them unable to concentrate their urine (low levels of adh (vasopressin), (vasopressin) anti-diuretic hormone) release from the pituitary gland.  
If you don't have this condition, then on test days avoid liquids (sips ok, glasses not ok) until after the urine test.  Best results would be to avoid liquids 4 hours before test.  If creatinine is still low, you might want to see an endocrinologist to evaluate for low adh (vasopressin). If the doctor can validate this, then your employer can't legally hassle you about your low creatinine.

You're fine: Especially if you're a bit more muscular than today's average guy, or a big guy, or you drank a lot of water that day, or it's actually a few more than 24 hours, or you ate an extra serving of meat, i'd consider this normal. The only value in measuring it is to give other substances "per gram of creatinine".

This is normal: Creatinine is normally excreted in the urine. The concentration of creatinine by itself means nothing other than the kidneys are doing their job. 

Blood creatinine will tell you better how well the kidneys are working.

: You are still confused about your renal function status so you are best answered face to face with a nephrologist. Urine creatinine in spot or random urine sample varies by the urine concentration and same with albumin in spot urine sample and that's why we use the ratio urine albumin/creatinine (UAC) so the result doesn't depend on the urine concentration. Normal UAC is less than 30.
