Diagnostic procedure: For older cardiologists like me physical exam after history often tells us what the cause and relative severity of the systolic murmurs is. The first test we generally order specific to the 'murmur' is the echocardiogram, this generally gives a very good indication of the valve problem and its severity. If necessary we might go on to other testing such as cardiac catheterization.

Unclear: Although you give some details from the exam, it's still not enough info. That description could be consistent with normal or it could be consistent with an atrial septal defect or other abnormality. If a cardiologist is concerned, an echocardiogram would likely resolve it.

my 12yo has had an innocent murmur since she was 1.5yo. they still heard it yesterday. rrr, nml s1&s2. noted 3/6 systolic ejection murmur heard best at left lower sternal boarder. is this concerning. i thought she would grow out of this by now?

: The description likely is what is labeled the innocent murmur of childhood. They may not "grow out of it" but many kids put on chest wall muscles or other tissue which will make the murmur less audible after the teen years. It's presence in the absence of related symptoms is not a problem.

Possibly: The most common cause is turbulence of blood crossing the aortic or pulmonic valves. Anemia (from malnutrition?) will exacerbate it. Innocent murmurs with no findings on echo can be systolic ejection murmurs.

Discuss with doc: If i listen to a kid often enough as they grow up i will eventually hear a murmur in most. Mild systolic ejection murmurs are often just flow  related, like the noise water makes in a babbling brook .Flow thru slightly narrow pulmonary arteries are frequent a cause of systolic murmurs. Clarify with your dr whether the murmur means anything. Most murmur"bad" is addressed at birth.

Murmurs...: An ejection systolic murmur is a murmur suring the systolic phase of the heart during the ejection through either the pulmonic or aortic valve (e.g. Aortic stenosis). This is different from a systolic murmur caused by regurgitation through one of the av valves.

Possibly: You are quite young and it is important to have regular follow-up.  The AR you describe may in fact be "trivial" and cause you no problems at all !   AR is typically a diastolic murmur - be sure to check with your doc about findings.  Many people live comfortably even with mild or moderate AR so be sure to address any risks for heart disease you may have and keep in touch with your doctor!

Any Murmur: With a diastolic component necessitates echo examination. Usually diastolic murmurs are associated with bicuspid aortic valves that do not completely close when they are supposed to, and result in backward flow of blood into the ventricle when the heart is relaxed. What you describe, though apparently mild, is pretty classic for that. Get an echo, I'm sure your doctor recommended one.

Soft murmur: This means that you have a very soft murmur that occurs at the tip of your heart ( the base of the ventricles). It occurs both in contraction (systole) and relaxation (diastole).  If you have had a heart cath and they did not tell you there was a problem with your valves then the murmur is likely not important.
