Stone in duct?: It's possible you have a calculus (stone) lodged at the tip of Warton's duct. The sublingual and submandibular salivary glands empty into Warton's duct, which runs beneath the tongue in the floor of the mouth. This is more likely the diagnosis if you feel a small hard lump in the same area in the floor of the mouth. This is called sialolithiasis. If the swelling persists see a physician.

Gland: Salivary glands typically swell from a blocked duct. The blockage can be a mucus plug which generally passes or a hard calcified plug which an oral surgeon can remove. It is common and I would suggest the oral surgeon first as this is an oral issue more than an ear nose or throat issue. Simple for the OS to diagnose.

Acute otitis: That is a very high fever and along with swelling (probably massive lymph node) your physician would likely check you for an acute otitis, mastoiditis, or even mumps. The lack of response to antibiotics after three days does not mean that the antibiotics failed though it might be worth discussing a change with your doctor, but it might also mean that the infection is viral and not bacterial.

Tonsillar abcsess: Infection of the tonsils can cause an abscess. This can lead to deviation of the uvula and is a serious infection which can cause airway compromise. I would recommend seeing a physician asap and if any symptoms of difficulty swallowing or breathing should be seen in the er.

Need to see physicia: Dear Abhishek: the appropriate treatment will depend on what is causing the enlarged glands (they are probably lymph nodes). You definitely need to see a physician to further evaluate the cause and give appropriate treatment. There are many different possible causes of enlarged lymph nodes and many do not require surgery. You may need longer: treatment with a different antibiotic. Have you tried flonase or rhinocort nasal spray,and how about antihistamines?
