Infection present: When this type of occurrence is present it is a sign that infection has resurfaced. It could be a reoccurrence of the original problem or it could be a new infection process occurring. I would advise that you have the area examined to determine the actual reason for this to be occurring. Your dentist will be able to provide you with an accurate diagnosis and treatment recommendations.

Seek dental consult: Redness and swelling in the mouth usually indicates infection. White spots often indicate an inflammatory condition referred to as a stomatitis, viral or bacterial in origin, that can spread to all areas of the mouth, and will  require anti-viral or anti-bacterial medication. All infections and lesions of the mouth should be examined immediately by a dental professional.

TMJ vs Salivary: This could be a couple of things, and you will most likely need to see a physician for relief. It could be an aggravation of TMJ syndrome, which would explain the difficulty opening and some swelling. Tmj is the joint you chew with. You could also have problem with a salivary gland, possibly even stone in the duct or an infection in the gland. Either way, you probably should make an appointment.

Second opinion: Amoxicillin is an antibiotic given to control infection, but you may need to see an oral surgeon or an endodontist (both dental specialists) to determine where the infection is coming from so that you can intercede

2 separate problems: 1. Neck and lower head pain. See your PCP.
2. Pain and swelling following dental extraction is usually at its worst 24 – 48 hours after surgery, after that it should subside more and more every day and after 7 days stop. Three month following surgery it's not OK. Time to call your dentist and to check the gum pain. Maybe another tooth needs attention?

: Only if you chew 24h/day, day and night, for months. Chewing gum can, however, stimulate your vagovagal reflex (look it up) which can leads to excessive stomach acid.
