partial palm, 4th, and fifth digit tingling began 10 weeks post rotator cuff surgery.  6 weeks later no change.  what is the connection?  what to do?

Brachial plexus: The shoulder joint is nearby the brachial plexus, and perhaps some nerve stretching occurred, during rehab, but the problem might be at the elbow instead.  Either way sounds in an ulnar nerve location.  An EMG might help.  See your doctor, as there very likely is some connection.

when i was arouind 6 i had an accident where i fell unto glass into my palm. i injured my ulnar nerve now my 4th and 5th digits are flexed with strength and muscle loss in the whole hand. is there surgery that can fix this or is it too late?

Can see hand surgeon: Old injuries to nerves many years ago generally are not fixable. Sometimes, surgery can be done to loosen (release) tendons or muscles that have contracted after an injury. Sometimes a hand surgeon can move a tendon or two to make the hand and fingers function better. One or two visits to a hand surgeon can answer such questions.

See details: Symptoms is those fingers indicate either an ulnar nerve compression at the elbow or wrist or compression of the 8th cervical nerve root in the neck. An evaluation and an EMG study will give you the answer.

Vasospasm: It appears that you're experiencing intermittent constriction of blood vessels in those digits. This can be due to cold exposure or spontaneous vasospasm. Raynauds phenomenon is a syndrome where coldness and color changes of the involved digits occur on exposure to cold.This  phenomenon can be part of an autoimmune disorder. Consult a rheumatologist for evaluation and advice.

Scar: Most likely scar tissue or cicatrix.  Other possibilities:  calcified vein, suture granuloma, cyst.  Less likely hematoma or abscess but anything is possible.

Dupuytren: Contracture is suggested on your description and among other conditions. Make sure you mention it to your prescribing doc for an eval for predisposing factors and further management once diagnosed.

Swollen hand veins.: Hand veins can swell for many reasons but the most common is certainly hereditary .  Other factors would be level of hydration, aging, sun exposure and loss of elasticity and volume. Actually, I am amazed at the amount of women that i see with bulging hand veins. Hand veins and especially palm hand veins are usually normal variations.  See a vein specialist if worried.

Usually: Seen more on the backs of your hands.  Palm skin is typically very thick.

Sorry,: But I am not aware of veins in the palm. Bulging vein on the back of your hands yes, it is seen during seniority when skin gets thinned and we are losing the fat pad around the veins. Than the veins look as they bulge more, 
the question is do you possibly confuse veins with tendons, as they are in the palm of your hand. If the hurt see a physician to rule out dupuytren's contracture!
