- how long ago was the initial melanoma diagnosed?
- how extensive are the lymph node and the distant metastases?
- is the melanoma in areas that can be treated surgically?
- are you healthy enough to get chemotherapy
some patients have a rapid decline, but others every 3-4 years have a new melanoma metastasis show up & are otherwise healthy.

: Each melanoma behaves differently. That’s the problem with cancers, that they don’t follow the rules of normal cells.Melanomas can grow very fast and, may have a very large component that is hidden to the naked eye and, spreading wider.
However, some patients with nodular melanoma evolve slower.
Impossible to say for sure. I recommend to have it fully treated once it’s diagnosed.It’s your priority

Once the melanoma has been surgically removed and the lymph nodes evaluated factors that will affect your treatment include the depth of the melanoma in mm, mitotic index, presence of ulceration. There is a new medicine which can be used but an oncologist will determine who is a good candidate.

But we must wait for the procedure to be done and the reprts will guide you. Melanoma can spread to any orghan in the body including thyroid but it is much less likely to be the case, as the benign thyroid nodules are a much more common occurence.

Both : Both the nausea and fever can be from an infection, especially with a white count of 22, 000. You really need to talk with your brother's surgeon to find the cause, which sometimes does remain hidden. Your brother has had fairly extensive surgery and his surgeon is the best person to determine where this infection is located. Dr. D makes some good points about other possible sources of these symptoms, however.

: Pancreatic cancer is one of the worst cancers as very few people survive it for more than 1-2 years.
Taking chemotherapy can buy extra time, may be an additional year or two.
I would have him see a medical Oncologist and take additional treatment; otherwise this tumor is likely to spread further within a matter of months.

Need more info: Sorry, but I am having trouble understanding your question. What type of skin cancer are you asking about? Common ones include Basal Cell, Squamous Cell and Melanoma. The word "metastatic" means that a cancer (any type) has spread to a distant location like a lymph node or another organ (lungs, brain). If a cancer is metastatic it is usually staged as stage 4, hence my confusion. Please re-ask!
