Never easy: Surgery or possibly radiation cure most of the throat cancers that are found early, but it is never a cinch. The likely outcome depends on the type of cancer and how far it has progressed. Death from throat cancer is as grim as you can imagine. I wish you luck and hope you stay proactive.

Trach care: I am sorry that your grandfather is not well. Trach care manuals for families may be found online. Keep the site clean and dry, humidification of the air keeps the passages moist and may lessen infection. Watch for signs of infection or trouble breathing and report changes to your physician.

Unknowable: Although the prognosis is grim (my sympathies), the question that should be asked is "how many joyful, meaninful days are left?" the answer lies in the love and support he derives from you, his family and friends.  Fortunately, to a large extent, the question can be answered by you and others who love your husband.

Chemotherapy: Mainstay of treatment for tumor that is metastatic is chemotherapy.  Local treatments like surgery, radio surgery or radiation can do just that local control.  So i would say chemotherapy + a local control if disease is limited.  Hospice and palliative care can be of great help if patient can not tolerate aggressive treatment and disease is advanced.

Not good : Stage 4 any cancer is not good, some better than others.  With head and neck cancers though unlike some others, cure is possible with aggressive mutimodality treatment.  Usually without any treatment the tumor grows locally, cause local symptoms and metastasize to lymph nodes and distant organs. Consider at least palliative treatment, it can improve quality of life for some time. 6-24 months range.

Know details: Unless surgery is emergency due to airway obstruction, most H&amp;N lesions which are squamous in origin are treated pre op with RT and if necessary chemo including Taxol (paclitaxel) and platinum.  If necessary high dose MTX used with leukovorum rescue.  Check to see if procedure is not tracheostomy to ensure airway during needed treatment.Lesions bestt reated with prior cytoreduction.

If you quit now...: ...The chances of you getting a whole bunch of nasty diseases (of which throat cancer is one) go way, way down.
