No clear prediction.: Each individual case is unique & predictions are very difficult--but from what you describe, the life expectancy is, unfortunately, not long. Large series show that median survival for patients with stage IV disease is 16 to 20 months in contemporary reports, and the five-year survival rate is less than 10 percent for patients with distant metastases. Kidney failure may worsen the prognosis.

So, what is the age, what other medical problems, what was the type of kidney cancer (clear cell renal cell, papillary, sarcomatoid, etc). There are molecular targeted therapies for specific tumor types....

Variable: There are many types of renal cancer, with different outcomes.  As your are 37 years old, renal cell carcinoma may be the type you are most interested in. Although not common, some individuals with stage 4 renal cell carcinoma have had prolonged (many years) of good quality life after diagnosis. I would recommend discussion with an oncologist about options for treatment.

Sunitinib Dosing: Sunitinib is usually given for 4 weeks, then 2 weeks off, and the response (reduction in tumor burden) assessed.  Often, the dose is reduced if side effects supervene.  There is no "standard" protocol and only your oncologist and the rest of your treatment team can recommend the best protocol for you.

Water retention: Your brother's kidney function i presume is not good, so water and urine are retained thus the swelling.
I presume he underwent chemo/radiation treatment.
Hemodialysis may help.

Depends on site: Survival will depend on location of metastasis, concomitant medical conditions, age, and  nutritional status.  Almost all patients with stage IV renal cell cancer are incurable. However, carefully selected patients may benefit from surgical resection of localized metastatic disease.  Only 8-10% of patients with stage 4 kidney cancer are alive at 5 years.

Improving: Formerly this was zero. A decade ago it might have been 10%.  With the new targeted medications for common renal cell carcinoma, it is impossible to give an accurate number since both the length and quality of survival are improving so surprisingly.
