Pinpoint is acute ms: Usually pinpoint pain is usually musculoskeletal in nature versus visceral. However, if you are having lung issues, a chest xray last year does not reflect your lung condition now. So, best to contact your doctor, and have him examine this, and advise you further what your next step is.  You may have strained your left shoulder blade, but only a physical exam will tell you this.

Nondiagnostic: This symptom is nondiagnostic and could be musculoskeletal or early pleurisy especially if aggravated by a deep breath and is accompanied by a cough. Esophageal spasm can cause upper back or front of the chest discomfort.  If such discomfort persists or worsens, get examined by a physician. Any appropriate xrays will be ordered.

Poor Posture: The most common cause is,poor,posture, with head and shoulders forward. This pulls scapula laterally, straining the rhomboids,,which hurt when you move the shoulder blades.  A good exercise is to place heels, buttock, shoulders and head against the wall, interlock fingers and place back of hands against the forehead, stretch elbows to wall.  Scapula thoracic syndrome
