This may be circulat: This  may be normal and related to her  normal physiology of how her  blood flow goes to the feet. Sometimes  we see  little fat pads  that can do this. Your  physician can help clarify this

Get it checked: Any form of "bottle decay" usually forms on the tongue side of the upper front teeth. What color is the stain? If it is dark and linear some infants get stain from taking iron supplements. If it is dark yellow, rarely and only rarely, infants get calculus (tartar) on there teeth and if they do, it is usually on the lower front teeth. Take your child to a pediatric dentist to get this evaluated.

: That's a somewhat unusual location for folliculitis; there are few hair follicles on the underside of the penis. If the "white spots" are blisters or pimple-like, get checked ASAP for herpes. Good luck.

Eczema: It could be eczema or warts, or an allergic reaction to something. Rashes are best treated in person so the doctor can see and feel to determine what it is.

: Since these spots, which are usually slighly raised, &amp; are on a hairless area of the penis they could be harmless Fordyce spots occurring from ectopic sebaceous glands within the skin of the penis. I believe a dermatologist would be best to evaluate and advise you.

Swollen ankles: can simply be from the flight and may resolve quickly on its own.  The red spots need to be evaluated by your doctor, especially after being in another country.  Please tell him/her about the ankles as well.  Peace and good health.

Whatever the initial: Cause of the rash, he probably scratched it, broke the skin & got a secondary bacterial infection. His pediatrician can open & culture one of the pustules to determine if it's a staphylococcal  infection & which oral antibiotic will cure it. Cut his fingernails very short. Give sitz baths with domeboro solution in 6" of warm water per pkg. Instructions. Give him the complete course of antibiotics.

i'm wondering if the rust spots on the bottom of this old tub are harmful to either me or my child, i can't afford a new tub. ?

No: The small amount of iron released into bath water is completely safe.  However, you should clean the rust spots and cover with appliance enamel to prevent the spots from rusting through and leaking.
