25 yr old f (not overweight) with chronic urq pain for 2+ yrs. no stones but hida showed 23 % function. unsure if removal is worth the risks, sod? greatly appreciate prof. opinions and likelihood of sphincter of oddi problems & seriousness of it.

Not very serious: The main problem with sphincter of oddi dysfunction is that it is an annoying pain that is intermittent and gets worse after eating. The treatment includes lifestyle changes (low fat diet), vasodilators like nifedipine, non opioid analgesics. Gastroenterologists can inject botox or make a cut at the sphincter of oddi and that works in more than 70% of the cases.

what can a hida scan find that an ultrasound cannot find?  aren't there multiple things other than gallbladder dysfunction? ultrasound was normal.

A lot.: After the tracer is injected into a vein, it must circulate to the liver. The liver must remove it from the blood and put it into the bile, liver circulation and function assured. The tracer, now in the bile, travels through the bile system and into the gall bladder, checking patency of these. A gall bladder stimulant can be used to measure gall bladder function and ejection out into bowel.

Don't know: What are the results of all the tests? This is best to be asked to the doctor who ordered the tests. If you do have a low gallbladder function, then sometimes gallbladder surgery relieves the problems. About 80% of patients get better & 20% do not improve with surgery.

Depends: It can be a problem but it is less certain that removal will lead to resolution of your symptoms. If your symptoms got worse during the hida, that is a good sign that you may have relief with cholecystectomy.

Reflux: Given the ultrasound is normal and HIDA did not show biliary dysfunction, I would not think a cholecystectomy would be of value.  However, you should address your reflux problem.

Sick GB: Nausea and only 9% function on a hida scan?  Sounds like a sick gallbladder is a distinct possibility.  Now whether you want surgery or not is up to you.  Depends on how miserable you are.  You may be able to keep symptoms under control with dietary changes.  The risk of not treating it, is that you might find that you need it removed emergently later. Not optimal.

An isotope: Tagged to a chemical compound is given iv. This compound is excreted in the bile and shows up when the patient is put under an imaging camera. Normally the bile ducts, gallbladder, and small bowel are seen during the course of the exam (about 90 minutes). If the gallbladder isn't seen or the compound doesn't flow into the bowel there can be gallbladder or bile duct disease respectively.

Ask the doc: Hida scan done to check for biliary obstruction (including in gallbladder). Generally not painful, but trying to pass gallstones is. Call your doc.
