Multiple features: Complex cysts, vs. Simple cysts, share common walls with other cysts, have solid components in the walls, or calcifications.

The radiologist will opine if features look suspicious and either recommend a repeat ultrasound in 6 months, or a ct to further clarify.

have possible hemorrhagic cyst on kidney .lesion middle kidney . having light bleeding . can this b a sign it's ruptured or what would cause this ?

Ans: There are many possibilities for the bleeding. First you must be sure it is not vaginal. If not then go to doctor and have a urine dip. Hydrate and see if blood resolves if not further testing is needed

Probably Not: AST/ALT can be elevated from several causes, including tissue and muscle injury. Moreover, the precise levels of these liver enzyme tests do not correlate well with the extent of liver problems or the prognosis. Trend in such values over a period of time would be helpful, along with interpretation by a m=physician experienced in such ailments.

Likely benign: While likely benign it is best to consult with your doctor to make sure it is not part of a more complex mass and to establish follow up imaging and referral to urologist if necessary.

These are common: The kidney is a bunch of tubes. They can get blocked, injured, plugged and can deform. The kidney can scar, can grow back some parts somewhat (and have injuries, scars, stones, infections, etc.). So..deformities are common and they change over time. BUT kidney cancer grows and is solid and irregular. 
U/S are grainy and can't get the same angle. Picture changes/thing changes/kidney changes.

how significant is the finding of protein in the urine in a child? if the child has a complex kidney cyst? repeat sample 1st morning urine showed none

Not very significant: Children frequently "spill" small amounts of protein in their urine when they are very active during the day from their high metabolism and rapid muscle growth. That's why we then check a first morning urine sample. If there is no protein with the morning urine, nothing to worry about at this time. I check a yearly urine sample on these kids, but otherwise no restrictions or reasons to worry.

Difficult situation: Us are quick but less specific with their findings, mri's are quite specific but take time and usually require sedation/anesthesia . Either  should be looked at in the context of the problem being studied. Either and or both can be used to follow a problem.Decide which doc you are more comfortable with and follow his/her advice, but continue to ask questions.

Anytime: Kidney cyst can already be present at birth, develop in childhood or as an adult. A simple kidney cyst is very common, in fact present in up to 1 in every 3 humans ; usually require no treatment unless they become symptomatic.

None: Usually no symptoms at all. Incidental finding in ultrasound. When they are very large may feel some discomfort in the flank area.
