But not everyone has it bad, so it is hard to know.

This is simple mendelian genetics, the old 1:2:1 ratio of outcomes if two carriers mate.  So if two sickle carriers mate, 1/4 offspring will be wild-type, 2/4 will be carriers, and 1/4 will have full-blown sickle cell disease.
