Many things: Your symptoms are very general.  This could be coming from your back (sciatica) or originating in your knee (?Arthritis).  If the symptoms have been present for more than a week, you should be evaluateed by your doctor.

Sciatica?: One possibility is a pinched or inflamed sciatic nerve, which runs down the back of the leg and through the gluts.  Please ask you wife to see her doctor.  If that's what it is, he/she may prescribe physical therapy or medication.  Peace and good health.  If something else, that will be identified also.

Differentiate: Between post traumatic femur pain and back pain. You may have injured something in your lumbar spine as well that could be contributing to nerve irritation, lending itself to shooting pains and restlessness. A good physical exam should be able to tell you which it is or if its a combo of both. Ask your physician about treatment modalities from pt to medications such as Gabapentin which can help.

: Best  approach to  "make it better" is to uncover  the cause.  See your doctor and get evaluated for potential peripheral neuropathy or  even a demyelinating  disorder.  This has no relationship to prior  heroin,  not used  for  10 yrs.

Nerve impingement?: An  impinged long nerve in your foot, when pressed could cause your symptoms of nerve pain running up your leg.  The source of impingement is the question: could be a cyst, a swollen ligament or tendon, a bone spur, or a calcification.  You need to get your PCP to take a look and refer you to an orthopedist for evaluation and imaging.

Spinal disease: It sounds like you REALLY need follow up with the neurologist or doc who did the procedure. Have you called the office? It's remotely possible you have a CSF leak after the spinal tap, although most heal on their own after a week of conservative treatment: strict bed rest, avoiding coughing, sneezing, nose blowing, and straining etc. You need a diagnosis to get helped. Hope you will feel better.

Radiculopathy: Description is consistent with nerve compression in the back, and this could be handled with physical therapy, exercise, even a brief course of oral steroids, and maybe local epidural steroid injection.  If disc desiccates and shrinks, could take 6-9 months.  If increase in pain, or weakness, and any bladder involvement, surgery would be best.  See a spine surgeon and discuss options.

i had a spinal block during can section a yr ago and since have had random shooting pain in arms& legs. what could this be?

2 different things: I am assuming your 'section' was a caesarian section and the block was placed in your lower back.  If so, shooting pains in your arms is absolutely unrelated since those nerves enter/exit the spinal cord in the level of your neck and upper back.  Because one thing follows another does not mean they are related: think "i washed my car and it rained".
