Middle left: There are many causes of one sided back pain:  localized muscle strain with spasm, facet joint irritation/inflammation is your most likely cause.  The facet joints is the site where the vertebral body meet in your back.
We ften recommend, rest, antiinflam meds, pt and it pain persist diagnositc and therapeutic facet injections/cortisone.
May need MRI and consult with pain physician.
